How to Use the Self-Evaluation Form
1. Pick Your Version
You can choose between:
- A blank version to fill out from scratch.

2. Download and Open the File
Download the file in the format you prefer—Word, PDF, or Excel. Open it in the right app, and if you’re asked to enable editing, go ahead and click that button.
3. Fill in Your Info
Start by adding:
- Your name
- Your role or position
- The date you’ll submit it to your manager
4. Rate Yourself
You’ll see a table with a "Self-Assessment Areas" column. This is where you can list the different performance areas you want to evaluate. For each one, choose the rating that best reflects how you think you did:

5. Share Your Thoughts

6. Save and Share
Once you’re done, save the file with a clear name like Self-Evaluation_YourName_ReviewPeriod.docx
