How to Use This Template
1. Download and Open the Template
Download the Excel file and open it in Microsoft Excel. Choose between the blank version or the sample-filled version.
2. Enable Editing
Click “Enable Editing” to activate the built-in formulas and allow data entry.
3. Fill in Employee and Evaluation Details
Enter the employee’s name, ID, position, department, evaluation period, and evaluator information.
4. Complete the Performance Criteria Section
- If you're using the blank version, enter your organization’s performance criteria.
- If you're using the sample-filled version, review and adjust the pre-filled content.
- Assign a rating and add comments for each criterion.
The Excel template will automatically calculate the Total Rating Score and Average Rating Score.
Tip: The Average Rating Score cell is color-coded for quick interpretation:
